  • 👈 Definition of disease:

Blockage of the narrow nasolacrimal duct, which naturally drains tears from the surface of the eye, is called lacrimal duct obstruction. It can be acquired or congenital, with the congenital type being examined in this topic.

👈 Causes of the disease:

Congenital lacrimal duct obstruction is common in children. 6-10% of children are born before the tear ducts are fully open. Some sources state that the prevalence of this disorder in newborns is 50%. The cause of this obstruction is the remaining membrane (curtain) at the end of the nasolacrimal duct on the nasal side.

👈 Symptoms of the disease:

Usually, babies with this problem show symptoms in the first 2 to 6 weeks after birth, the most common of which is tearing. In the first month after birth, we do not have a reflex tear secretion (tears) when crying, and if tears do come, they are most likely pathological (caused by a disease). Other symptoms of this disease include discharge, recurrent conjunctivitis and inflammation and infection of the lacrimal sac, in which case the area under the eye, near the nose, becomes red and inflamed and sensitive to the touch. This area may swell and mucous secretion can be seen on the inner side of the eye. In 1/3 of cases, this involvement is bilateral. Usually, the baby's symptoms are aggravated by dust and wind, but there is no fear of light (photophobia). Note: In children who have tearing and photophobia, congenital glaucoma should be considered.

During lacrimal duct cannulation, after dilating the initial point of the lacrimal duct and performing lacrimal duct irrigation, the lacrimal duct is rinsed to ensure the success of lacrimal duct irrigation. To maintain patency of the lacrimal duct, lacrimal duct intubation is recommended.
In 1970, the placement of a Silastc tube was first described to prevent lacrimal duct re-stenosis.
Lacrimal duct intubation in children with congenital lacrimal duct obstruction is a standard procedure in patients in whom lacrimal duct irrigation has failed.
Silicone tubes, which are less invasive to the lacrimal duct, are now available.
Silicone tubes can be easily left in the lacrimal duct for long periods of time.
Silicone tubes are easily tolerated and do not cause any irritation, these silicone tubes have minimal irritation and displacement in the tear duct. Silicone tubes can be easily removed after ensuring that there is no remaining blockage or stricture in the tear duct.
